Our evidence-based assessments are designed to provide clarity and direction, empowering you and your family to make informed decisions about your care. To meet you where you are, we offer three flexible options for assessments:

During your assessment, our team will work closely with you to understand your needs, challenges, and goals. Based on the findings, we’ll recommend the services that best align with your situation. Whether it’s therapy, case management, or another program, we’ll help you take the next steps and schedule your initial appointment.

Take Your First Steps
Our comprehensive, evidence-based clinical intake involves five key steps:
- Reception: Begin by providing basic contact information.
- Financial Intake: Review your insurance details or qualify for our sliding fee scale if uninsured. For clients under 18, additional funding options may apply.
- Clinical Intake: Meet with a clinician for a full mental health and substance use evaluation.
- Follow-Up: Receive follow-up appointments and emergency contact details after completing the intake.
- Flexible Timeline: While the full process may take 2-4 hours, it doesn’t have to be completed in a single visit.
What to Bring:
- Picture ID
- Social security card
- Insurance card (we accept all insurances)
- Proof of income (pay stub, W-2, SSI, unemployment, etc.)
- Guardianship documentation (if applicable)
Protecting Your Health
In deciding whether to schedule an in-person or telehealth, please be mindful of any symptoms you may have that could spread viruses to others. Please help us protect you and others from the spread of respiratory viruses. If you have a fever or chills, a cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, congestion or runny nose, diarrhea, or other symptoms that might indicate you have a respiratory virus, please do not schedule an in-person appointment. If you’re experiencing symptoms, please avoid scheduling an in-person appointment. Instead:
- Opt for a telehealth appointment
- Reschedule your visit for a later date

Same-day Assessments are available at the Langhorne Wellness Center. We strive to provide same-day assessments as quickly as possible. Wait times may vary depending on demand, but we recommend arriving early in the day for the best chance of being seen promptly. For more information or to check availability, call 434-477-5000.
There are five steps to our comprehensive, evidence-based clinical assessment process.
- Client meets with reception/administrative assistant to start collection of basic contact information.
- Client meets with a member of our financial intake team to review insurance information. If a client does not have insurance, the ability to pay scale is utilized. If the client is under 18, additional funding may be available and will be reviewed.
- Client meets with a clinician to complete the clinical intake, which is a comprehensive mental health and substance use diagnostic evaluation, to open the client to services at the agency.
- The process may take from 2 - 4 hours, but all steps do not have to be completed in the same day.
- Once the intake process is complete, the client will receive follow-up appointments, as well as emergency contact information if needed.
When you are ready for services, please bring a picture ID, social security card, insurance card (we accept all insurances) and proof of income (such as a paycheck stub, W-2, SSI, unemployment, etc.). Please also bring any guardianship documentation if that applies.
